WOW! what a win! Love this idea and will be working to try to get this here. We send out devel surveys before well checks, but parents usually forget them. LOVE the printing at check-in idea.
Innovative approach to use Pre Visit Summary to help get patient to enter questionnaire. Did a good job of fitting the functionality into the broader spectrum of tools (MyC Patient Entered Qnr and SmartForm) and made a good case that we will continue to need some paper based screening. Articulate presenters. Great artwork.

What changes will you apply . . .
. . . Work with pediatricians to add this to their ambulatory epic
. . . Implement a new strategy
. . . Will consider transitioning from ASQ to SWYC if integrated with Epic foundation 2018.

. . . Review the current workflows to determine efficiency and see where some of these suggestions could be used.

. . . We currently have the M CHAT R and Pediatric Symptom Checklist built in an electronic, patient facing questionnaire - prompted based on age. We are seeking an electronic, patient facing option for completing developmental screening. Until the SWYC is approved for use by the Mn Dept of Health, or another electronic option is available, we will continue with our current process (ages & stages questionnaire). We will look at the build for adjusting for corrected age - our current workflow is much more manual in that regard.
